PRIVACY POLICY STATEMENT.
You have signed an application form to become a member of S.D.S.R.I. This Privacy Statement relates to the collection, maintenance, use and disclosure of personal information about you and provided by you and in all future communications between you and S.D.S.R.I. directly or indirectly.
1. Collecting Organization and what information we collect.
· In its capacity as an affiliated branch of NSW Soccer Referees Inc., S.D.S.R.I. collects personal information. Such information may include name, address, telephone number, email address and grading status.
· S.D.S.R.I. also maintains a member’s register, which includes personal information as summarised in the dot point above.
2. Purpose of collecting information.
The primary purpose of collection of personal information is the maintenance of the register of members to facilitate registration with NSW Soccer referees Inc., for insurance purposes, for appointment of games, for communication with members for such appointments (either by telephone, mail or email) for communication between members to facilitate the provision of car pools for travelling to games, for facilitating communication between members to foster greater dialogue between members for discussion of laws of soccer, re-grading of members etc.
3. Consequence of not providing information.
If you do not provide complete and accurate information, S.D.S.R.Inc., may not be able to effectively maintain its register of members. If we do not get the personal information required, we, or NSW Soccer Referees Inc. may not be able to affiliate you and consequently you will not be covered by insurance to enable a member to carry out duties associated with soccer refereeing.
4. Disclosure of information to other organizations.
Your personal information may be disclosed to NSW Soccer Referees Inc., to the insurance underwriter, to New South Wales Amateur Soccer Federation/ New South Wales Soccer Federation (or to their successor), Southern Districts Soccer Football Association Inc, to members seeking to inspect the register of members, regulatory bodies, including the Department of Fair Trading, NSW Police and the Australian Taxation Office.
Your personal information may also be disclosed to members of the Management Committee or co-opted persons or contracted external service providers for the purpose of up-loading information into the accounting system managed by the Treasurer of S.D.S.R.Inc., mailing of annual reports, notice of meetings and other information that may be sent form S.D.S.R. Inc. or NSW Soccer Referees Inc., to its members.
5. Legal requirements.
When providing services to members, S.D.S.R.Inc., is obliged by law to collect and disclose personal information from you. The laws governing these requirements include:
The Associations Incorporations Act.
The Financial Transactions Report Act.
Australian Taxation Laws.
Working with Children Act.
Social Security Act.
6. Posting of electronic images on the S.D.S.R. Inc. web site.
S.D.S.R.Inc. has a web site at www.sdsreferees.com.au for the provision of Internet and Intranet content to members and visitors from the worldwide web at large. S.D.S.R.Inc. will from time to time post electronic images of its members on the web site. The member will be identified by their first name only. Unless advised to the contrary, permission is deemed to be received from you for the up load of an electronic image of you. Members who do not wish to participate in the posting of their electronic image on the web site are to contact the S.D.S.R.Inc. webmaster.
7. Access to information.
Under the National Privacy Principles, you can access personal information that S.D.S.R. Inc. holds about you. You also have the right to request S.D.S.R.Inc to correct information about you, which is inaccurate, incomplete or out of date. To ensure the integrity and security of members’ information, S.D.S.R.Inc will only disclose to members such information if internal procedures governing security are satisfied.
